Payment Policy

To ensure complete, quality and individualized medical care Mountain Sage is not contracted with health insurance companies. It is Dr. Silapie’s  belief that proper health care procedures and treatments should be determined between a physician and you, the person seeking heath care.  The way you proceed with your health is ultimately your decision. For your convenience we will determine out-of -network insurance coverage and submit claims on your behalf based on your allowed benefits. Full payment is due at the end of each visit, regardless of insurance coverage.  If you need to reschedule please inform our staff at least 24 hours prior to your scheduled appointment to avoid charges.   Cash, Check, Visa and Mastercard are currently accepted forms of payment.
